A RESOLUTION APPROVING A FINAL DEVELOPMENT PLAN FOR THE COLORADO
BOULEVARD MEDICAL AND PROFESSIONAL CENTER
WHEREAS, there has been submitted to the Board of Trustees of the Town of Firestone a
request for approval of a final development plan for the Colorado Boulevard Medical and
Professional Center; and
WHEREAS, all materials related to the proposed final development plan have been
reviewed by Town Staff and the Firestone Planning and Zoning Commission and found with
conditions to be in compliance with Town of Firestone subdivision and zoning ordinances,
Development Regulations, and related Town ordinances, regulations, and policies; and
WHEREAS, the Firestone Planning and Zoning Commission has held a properly noticed
public hearing on the application, and has forwarded to the Board of Trustees a recommendation of
approval, with conditions; and
WHEREAS, the Board of Trustees has duly considered the proposed final development plan
and has held a properly noticed public hearing on the application; and
WHEREAS, the Board of Trustees finds that the proposed final development plan should be
approved subject to certain conditions.
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF TRUSTEES OF THE
TOWN OF FIRESTONE, COLORADO:
Section 1. The Board of Trustees of the
proposed final development plan for the Colorado
subject to the conditions set forth on Exhibit A,
reference.
Town of Firestone does hereby approve the
Boulevard Medical and Professional Center,
at hereto and incorporated herein by
INTRODUCED, READ AND ADOPTED this 28t" day of May, 2014.

EXHIBIT A
Final Development Plan
Conditions of Approval
Colorado Boulevard Medical and Professional Center
1. Modify documents pursuant to comments from the Town Engineer.
2. Revise the FDP to note the color of the dumpster will match the brick veneer of the main
building.
3. Provide an updated title commitment.
4. Revise the FDP as necessary to reflect correct ownership at the time of execution and
recording.
5. Revise Sheets 9 and 10 to include proper title blocks and the Firestone Information
Block,
6. Note that any change in location of the monument sign shall be subject to approval by the
Town Engineer and Town Planner,
7. Correct the title block on Sheet 5 and other locations as necessary to identify the proper
project name.
8. Remove the language regarding decks as they do not appear appropriate for this building,
or provide information to clarify potential locations, materials and colors.
9. Clarify the color and materials of the monument sign, or add a note that the colors and
materials of the sign shall be similar and complementary to the main building.
10. Modify the land uses table to state the actual number of parking spaces for the site.
