Dear Union North LLC,
In accordance with the requirement pursuant to Rule 412.a.(4) of the Rules and Regulations of the Colorado
Oil and Gas Conservation Commission (“COGCC”), PDC Energy, Inc., (“PDC”), as Operator, hereby gives
written notice of its intention to conduct operations on the above referenced lands no sooner than seven (7)
days from today’s date.
PDC intends to bring a Workover Rig on to the above described property to work on the SRC Union A-
5CHZ well(s). Please see the attached map of the existing well for reference. If you have any questions or
would like to arrange a site visit, please do not hesitate to give me a call at (970) 373-6323.
As surface owner you have the responsibility for notifying any affected tenant of the proposed operations.
Please have them contact me if we need to coordinate around any crops in the area in order to prevent damage
if possible. Thank you for your cooperation in this matter.
