This form is to be submitted as an Intent to Abandon whenever an abandonment is planned on a borehole. After the
abandonment is complete, this form shall again be submitted as a Subsequent Report of the actual work completed. The
approved intent shall be valid for six months after the approval date, after that period, a new intent will be required.
Attachments required with the Intent to Abandon are wellbore diagrams of the current configuration and the proposed
configuration with plugs set.
A Subsequent Report of Abandonment shall indicate the actual work completed. Attachments required with a Subsequent
Report are a wellbore diagram showing plugs that were set and casing remaining in the hole, the job summaries from all
plugging contractors used, including wireline and cementing (third party verification) and any logs that may have been run
during abandonment.

COA Type Description
Confirm existing J-Sand plug with tag. Contact COGCC if deeper than 7734'.
Verify existing cement coverage by CBL - submit to COGCC for verification of plugging
orders prior to continuing plugging operations.
1) Provide 2 business day notice of plugging MIRU via electronic Form 42.
2) After placing the shallowest hydrocarbon isolating plug (6976'), operator must wait a
sufficient time to confirm static conditions.
3) After placing plug at 3200¶assure that all fluid migration has been eliminated by
monitoring the well for a minimum of 8 hours before proceeding to the next plug. If at
any time after placing this plug there is evidence of pressure or of fluid migration,
contact COGCC engineering before continuing operations.
4) Prior to placing the 580¶plug: verify that all fluid migration (liquid or gas) has been
eliminated. If evidence of fluid migration or pressure remains, contact COGCC Engineer
for an update to plugging orders.
5) After isolation has been verified, pump plug and displace. If cement is not circulated
to surface, shut-in, WOC 4 hours then tag plug ±must be at 212¶or shallower and
provide at least 10 sx plug at the surface.
6) Leave at least 100¶of cement in the wellbore for each plug.
7) Properly abandon flowlines as per Rule 1105. Pursuant to Rule 911.a. Closure of Oil
and Gas Facilities, Operator will submit Site Investigation and Remediation Workplans
via Form 27 for COGCC prior approval before cutting and capping the plugged well,
conducting flowline abandonment, and removing production equipment. Pursuant to
Rule 1105.f. Abandonment Verification, within 90 days of an operator completing
abandonment requirements for a flowline or crude oil transfer line, an operator must
submit a Field Operations Notice, Form 42-Abandonment of Flowlines for on-location
flowlines, and a Flowline Report, Form 44, for off-location flowlines or crude oil transfer
lines.
8) With the Form 6 SRA operator must provide written documentation which positively
affirms each COA has been addressed.
Prior to starting plugging operations a bradenhead test shall be performed if there has
not been a reported bradenhead test within the 60 days immediately preceding the start
of plugging operations.
1) If, before opening the bradenhead valve, the beginning pressure is greater than 25
psi, sampling is required.
2) If pressure remains at the conclusion of the test, or if any liquids were present during
the test, sampling is required.
The Form 17 shall be submitted within 10 days of the test. Sampling shall comply with
Operator Guidance - Bradenhead Testing and Reporting Instructions. If samples are
collected, copies of all final laboratory analytical results shall be provided to the COGCC
within three (3) months of collecting the samples.

procedure.

1. Operator will implement measures to control emissions to protect health and safety,
and to ensure that vapors and odors from well plugging operations do not constitute a
nuisance or hazard to public health, welfare, and the environment.
2. Prior to commencing operations, at a minimum, operator will contact the
neighborhood HOA and offer information regarding the nature and timing of the P&A
operations.
3. Prior to commencing operations, at a minimum, the operator will post signs in
conspicuous locations coordinated with the neighborhood HOA or City of Firestone. The
signs will indicate plugging and abandonment operations are being conducted, the well
name, well location information, and the Operator's contact information. Signs must not
create a traffic hazard.
4. Prior to commencing operations, at a minimum, the operator will provide an
informational sheet to the Building Units that are adjacent to the parcel on which the
well is located. The sheet will include the Operator's contact information, the nature and
timing of the P&A operations.
5. This oil and gas location is within a CPW-mapped bald eagle roost or communal
roost High Priority Habitat. The P&A activities must not take place between November
15 to March 15 if there is direct line of sight between the active roost to the plugging
activity. If plugging activities take place from November 15 to March 15, the Operator
must coordinate with CPW in advance and conduct a pre-plugging roost survey to
assess roost activity and distance from the roost to the location.
6. This oil and gas location is located in the proximity of CPW-mapped High Priority
Habitat for Aquatic Native Species Conservation Water. Operator will review stormwater
program and implement stormwater best management practices or erosion control
measures as necessary to prevent sediment or impacted stormwater from entering
Boulder Creek.

MIRU cementers. Pump Fox Hills Balance Plug: Pump 50 sx (10.3 bbl or 58 cf) of the Neat G Cement (15.8 ppg & 1.15 cf/sx). Volume
is based on 580' in 4-1/2", 10.5# production casing with no excess. The plug is designed to cover 580'-0'. Plug length exceeds 500'.
Consult with Foreman or Engineer on splitting up the plug. Verify and document cement to surface. Collect wet and dry samples of
cement to be left on rig.
27 COA: If cement was not circulated to surface, then WOC 4 hours. Tag TOC. TOC must be 25' or shallower. If tag is too deep or
there is evidence of pressure or fluid migration, contact Engineering.
28 Pull out of cement. TOOH, LD all 2-3/8" tbg. PU and TIH with 16' of tbg subs. Top off cement to surface. Circulate clean. TOOH
and LD tbg subs. RDMO cementers. ND BOP. Install night cap. RDMO WO rig.
29 Instruct cementing and wireline contractors to e-mail copies of all job logs/job summaries to rscDJVendors@anadarko.com within
24 hours of completion of the job.
30 Supervisor submit paper copies of all invoices, logs, and reports to VWP Engineering Specialist.
