The Flowline Report, Form 44, shall be submitted to register, report realignment, report removal from service, provide pre-
abandonment notices, or report abandonment of Off-Location Flowlines, Flowline Systems, Produced Water Transfer Systems, or
Crude Oil Transfer Lines or Systems as required by the 1100 Series Rules. The Form 44 shall also be submitted to register, report
realignment, or report abandonment of Domestic Taps, and to report Grade 1 Gas Leaks from Flowlines per Rules 610 and 906.

Description of Corrosion Protection:
Crestone pipelines are covered in a protective external coating. Flowlines are subject to thorough inspections and
fabrication standards during installation to minimize coating and pipe defects. Crestone maintains a chemical program
to reduce internal corrosion. If a flowline is found to lack integrity, Crestone will investigate, report, and remediate any
spills in accordance with the 900 series rules.

Crestone flowlines are pressure tested prior to placing flowlines into service. Flowlines are tested on an annual basis
in accordance with COGCC 1104 series rules. If a flowline is found to lack integrity, Crestone will investigate the root
cause, as well as report and remediate any spills in accordance with the 900 series rules. All repairs will be completed
in accordance with COGCC 1102 Series rules.
Description of the construction method used for public by-ways, road crossings, sensitive wildlife habitats,
sensitive areas, and natural and manmade watercourses (i.e., open trench, bored and cased, or bored only), if
applicable.
The majority of flowlines are not near any known and defined public receptors or road crossings. Those that are,
county permits are acquired prior to construction of county road crossings. Permit conditions contain minimum
construction requirements.
FLOWLINE SYSTEM PRE-ABANDONMENT NOTICE

Description of Pre-Abandonment Notice:
The following off-location flowlines will be partially abandoned in place:
The flowline serving the IONE 21-2 (05-123-20717) will be partially abandoned in place as per Rule 1105.d and
verified by a third party. The flowline segment that runs under, as well as alongside other active flowlines, will be
abandoned in place, and the remainder of the flowline will be removed. See attached exhibit for flowline path.
Where the flowline segments are removed, the trench will be backfilled and the land surface will be graded.
The flowline serving the IONE 11-2 (05-123-20721) will be partially abandoned in place as per Rule 1105.d and
verified by a third party. The flowline segment that runs under, as well as alongside other active flowlines, will be
abandoned in place, and the remainder of the flowline will be removed. See attached exhibit for flowline path.
Where the flowline segments are removed, the trench will be backfilled and the land surface will be graded.
Pre-Abandonment 30-day Notice
Removed per Rule 1105.d.(2)
Abandoned In Place per Rule 1105.d.(2) Exceptions - select all that apply:
A. A surface owner agreement executed by a surface owner allows abandonment in place.
B. The line is subject to the jurisdiction of the federal government, and the relevant federal agency directs
abandonment in place.

C. The flowline or crude oil transfer line is co-located with other active pipelines or utilities or is in a recorded right of way.
D. Removal of the line would cause significant damage to natural resources, including wildlife resources, topsoil,
or vegetation.
E. The flowline or crude oil transfer line is in a restricted surface occupancy area or sensitive wildlife habitat.
F. The flowline or crude oil transfer line or a segment of the line crosses or is within 30 feet of a public road,
railroad, bike path, public right of way, utility corridor, or active utility or pipeline crossing.
G. The flowline or crude oil transfer line or a segment of the line crosses or is within 30 feet of or from under a
river, stream, lake, pond, reservoir, wetlands, watercourse, waterway, or spring.
H. The operator demonstrates and quantifies that the removal of the flowline will cause significant emissions of
air pollutants.
Form 44 filed to report PRE-ABANDONMENT notification. The following flowlines will be partially abandoned in place:
12320717_FL: services the IONE 21-2 (05-123-20717)
12320721_FL: services the IONE 11-2 (05-123-20721)
12320717_FL and 12320721_FL do not have any spill/releases reported on a Form 19.
GIS data has previously been submitted. It is anticipated that abandonment operations will start on or about
xx/xx/2024.
All other flowlines in this system will remain as previously reported.
