How was extent determined?
Excavation was conducted in order to determine the extent. Impacted soils were removed and transported to a disposal facility.
Transport and disposal records will be kept on file under usual and customary practice and are available upon request. Soil samples will
be collected and analyzed for COGCC Table 915 constituents of concern until the areal and vertical extents of the excavation are within
allowable limits.

Describe what is known about the spill/release event (what happened -- including how it was stopped, contained, and recovered):
Surfacing of produced water evident at the surface leg of the produced water side of the separator. Water leg isolation control valve is
shut. It is believed that all contamination is confined within secondary containment. A work order has been submitted to have the
contamination cleaned up.

Produced-water was found to be surfacing adjacent to the produced-water side of the separator inside secondary containment. The
water leg isolation control valve was shut. Produced-water and potentially impacted soils were hydrovacced out. Only preliminary lab
data is available at this time and the investigation activities are ongoing.
Describe measures taken to prevent the problem(s) from reoccurring:
The failed equipment has been shut in and will be repaired. Equipment will be monitored according to routine maintenance intervals.

report.
