This is an environmental inspection in response to a Colorado Environmental Spills/Releases Reporting Form (Record ID 20230604
(Incident Report Number: 2023-0604) attached. The form describes a potential spill at an Oxy location adjacent to a detention pond in the
Town of Firestone. Upon inspection it appears it appears the location described in the Spills/Releases Reporting form is the
JUNCTION-62N68W 2NWSW location (Location ID 336314) which is located adjacent to the location where the GPS coordinates
provided plot.
As such an environmental inspection was performed for each of the following:
-JUNCTION-62N68W 2NWSW location (Location ID 336314)
-JUNCTION 22-2 well (API 123-27367)
-JUNCTION 12-2 well (API 123-27377)
-JUNCTION 11-2 well (API 123-27376)
-Junction 11, 12, 22-2 O SA Facility (Spill/Release ID 484183)
-Remediation Project #27339/Release Point ID 484183 (Open) - For decommissioning of the tank battery and a release associated with
historic impacts associated with the northeastern AST.
-Remediation Project #27343 (Open) ±For plugging and abandonment of the Junction 12-2 and 22-2 wellheads and the removal of the
flowlines, and the removal of the flowline associated with the Junction 11-2 wellhead.
Photos attached document site conditions at the time of this inspection.
Any corrective actions from previous inspections and/or conditions of approval from previous forms that have not been fully addressed are
still applicable.

Tracking Pad Fail Vehicle trackout was
observed onto CR 2450.

Comment: -Vehicle trackout was observed onto CR 2450. Trackout pad requires maintenance
-No BMPs are in place to control stormwater runoff in a manner that minimizes erosion,
transport of sediment offsite and site degradation. Northern perimeter of the pad is not
stabilized and is showing signs of rill erosion and transport of sediment/pad material off
location.
-Stormwater run-in to the open excavation has resulted in rill erosion, wall destabilization
and transport and accumulation of stormwater and sediment within the open excavations.
Corrective Action: Per Rule 1002.f., oil and gas operators shall implement and maintain Best Management
Practices (BMPs) at all oil and gas locations to control stormwater runoff in a manner that
minimizes erosion, transport of sediment offsite, and site degradation.

BMPs shall be maintained until the facility is abandoned and final reclamation is achieved
pursuant to Rule 1004. Operators shall employ BMPs, as necessary to comply with this
rule, at all oil and gas locations, including, but not limited to, well pads, soil stock piles,
access roads, tank batteries, compressor stations, and pipeline rights of way. BMPs shall
be selected based on site specific conditions, such as slope, vegetation cover, and
proximity to water bodies, and may include maintaining in-place some or all of the BMPs
installed during the construction phase of the facility. Where applicable based on site-
specific conditions, operators shall implement BMPs in accordance with good engineering
practices.
